    (d) Disclosure of names and addresses of customers.

    (8) PS Form 1583, Application for Delivery of Mail Through Agent.
Information contained in PS Form 1583, Application for Delivery of Mail
Through Agent, may not be disclosed to the public, except as follows:
    (i) For the purpose of identifying a particular address as an
address of an agent to whom mail is delivered on behalf of other
persons. No other information, including, but not limited to, the
identities of persons on whose behalf agents receive mail, may be
disclosed from PS Form 1583.
    (ii) When the delivery address is being used for the purpose of
doing or soliciting business with the public, as indicated on PS Form
1583 or by other evidence furnished by the requester (such as an
advertising circular). Disclosure will be limited to the recorded
business name, street address, and telephone number of the addressee.
When the postmaster is unable to determine whether a business use is
involved, he shall refer the request to managing counsel of the
appropriate field legal office for advice. Only if the addressee's
business and home address are the same, will the home address be
provided pursuant to this provision.
